If your doctor request you undergo one or more of the following diagnostic test(s), therapy and/or treatment:
Please call your insurance carrier to verify whether you need pre-certification for any of these procedures, tests and/or medications your doctor has prescribed for you. If you do require pre-certification, please contact our office at (212) 305-0622. Our office staff will obtain all necessary authorizations through your insurance carrier.Please be advised that it may take up to 3 business days to obtain authorizations/pre-certification. Once the authorization/precertification is obtained, our office staff will contact you with the information and any necessary information you will need regarding scheduling your test and/or series of injections with the doctor. Remember to bring your requisition/prescription for the test with you on the day of your test. |

What If I Need To Reschedule My Test Appointment? |

If you are unable to keep your scheduled testing appointment, please contact the ancillary facility directly to reschedule. You might also need to contact our Appointment Scheduling Office to reschedule your appointment with your doctor to ensure that the test results will be available for your visit with the doctor. |
